Why change amalgams

A long time ago we decided not to use more amalgams for different reasons:

  • Toxic to patients by (dissolving in saliva) and by inhalation upon evaporation.
  • Toxic for those who manipulate it (auxiliaries and dentists).
  • Little working time (hardens in a few minutes).
  • Unsightly dark color.
    Difficult handling.
  • We incorporated metals in the mouth and interfered with the body’s energy current.
  • Poor anatomical results, very difficult to shape naturally.

If the patient has technically correct old fillings of amalgam, without caries, without leaks, without sensitivity, without fractures or fissures. We will assess the size, aesthetic importance for the patient and we will balance the risk of having postoperative sensitivity with the risk posed by heavy metals in the mouth and we will make a decision.

On December 1, 1992, at the Spanish Dental Association, Dr. Jorge Vanni Navarro presented the progressive composite technique of Dra Lopez – Dr Vanni based on the progressive waxing technique used in the dental laboratory to naturally shape crowns or dental caps. With this technique we can achieve results like those shown in some of our clinical cases.

Amalgam extraction protocol

Keep the amalgams cold.

All removal should be done under cold water spray with copious amounts of water. Once removal has begun, the mercury vapor will be continuously expelled from the tooth.

High volume sucker.

A tip of the high volume suction cup will be placed close to the tooth at all times to evacuate steam from this patient area. Polishing the amalgam can create very dangerous levels of mercury and should be avoided especially for the mercury poisoned patient.

Alternative air source

All patients who have an amalgam removed or placed should be provided with an alternate source of air and instructed not to breathe through their mouth during treatment. A nasal mask such as the one used with nitrous oxide anesthesia equipment is excellent.

Immediate removal of the mercury alloy.

Mercury alloy particles must be washed and aspirated as soon as they are generated. The filler should be sectioned and removed into large pieces to reduce exposure.

Gloves washing and changing.

After the fillers have been removed, remove the rubber dam if used and wash the patient’s mouth for at least 30 seconds with cold water and suction, we will remove the gloves and replace them with a new pair.

Clean the patient immediately.

Immediately change the patient’s protective clothing and clean their face.

Safe Amalgam Extraction

  • We always isolate with rubber dam or optradame.
  • Gold filters to protect the patient against mercury vapors.
  • Filters against mercury vapors for auxiliaries and Drs.
  • Metal filters in dental chair aspiration systems to protect the environment.
  • We use amplification systems (Loupes and Zeiss Omni Pico Microscope).
